How do I get my service credit rebate?

To receive your rebate: simply fill out our online form below, click on the Next button and send the printed form to our office by postal mail. As an alternative, you can drop your completed rebate form off at our office.

Incomplete forms or forms with errors will not be processed. Early termination fee of $40 applies to any promotional contracts.

Rebate form must be received by our office within 30 days of your Cable Modem installation. Approximately 6-8 weeks after receipt, your service credit will be applied.
